Our Social workers focus on the safety of our patients while supporting families during some of the most difficult times. Saying goodbye to our loved ones can be heartbreaking and as social workers we strive to be that extra support wherever needed. We provide resources, education links for caregivers to get support. Our social workers ensure patient comfort and care as well as helping families cope with personal, emotional and social challenges. Grief therapy, counseling, are also offered and our empathetic nature to help our families is what we strive for. Working with a team of professionals we collaborate on a care plan and adjust throughout the stages of Home Health or Hospice.
