Who Pays for Hospice Care?

In most cases, hospice care is entirely covered by Medicare, with minimal out-of-pocket expenses for patients and their families. Additionally, private health plans and Medicaid also offer coverage for hospice services, ensuring that individuals facing terminal illnesses have access to the care and support they need during difficult times. Hospice Coverage Through Medicare

The Medicare Hospice Benefit aims to provide access to services that address the physical, emotional and spiritual needs that come with a terminal illness.

To qualify for the Medicare Hospice Benefit, patients must meet these requirements:

  • Be eligible for Medicare Part A (hospital insurance). Check if you are eligible at medicare.gov
  • Have a physician certify that their life expectancy is 6 months or less if the terminal illness runs its natural course
  • Wish to receive palliative care focused on comfort rather than curative treatments

It's important to note that hospice care can be stopped at any time if the patient's condition improves or they wish to resume curative treatments. As long as they continue meeting eligibility criteria, Medicare beneficiaries can re-enter hospice care. Hospice coverage also extends beyond 6 months if needed, with physicians periodically re-certifying the prognosis.

Under the Medicare Hospice Benefit, patients typically have very minimal out-of-pocket costs. Covered hospice services include:

  • Visits from the interdisciplinary hospice team
  • Medications for pain/symptom management
  • Necessary medical equipment and supplies
  • Short-term inpatient and respite care
  • Spiritual care and grief counseling

Private Health Insurance Plans

The majority of private health insurance plans also provide hospice coverage in line with Medicare benefits. Coverage details vary by plan, but often include services such as:

  • Routine home care visits
  • Continuous in-home care (during crisis periods)
  • Inpatient respite care
  • Hospice care in a facility or hospital

Hospice Coverage Through Medicaid

For those who qualify, Medicaid hospice benefits are very similar to Medicare. The services covered include physician services, nursing care, counseling, medications, supplies, therapies, and more. Medicaid hospice eligibility varies by state.
