What is Hospice Care?

Hospice care is a specialized form of care provided for those with life-limiting conditions that give them a life expectancy of 6 months or less, as determined by a physician. Rather than seeking to cure the disease, hospice care shifts the focus to managing symptoms and increasing comfort as the illness progresses naturally. This compassionate approach allows patients to live fully during their remaining time.


How Hospice Care Works at Amavi

When Amavi Hospice is invited into the patient's home, a dedicated team of caring professionals collaborates to create a personalized care plan tailored precisely to the patient's needs, values, and wishes. Our interdisciplinary hospice team includes:

  • Hospice Doctors and Nurses to expertly manage pain, symptoms and provide skilled medical care.
  • Hospice Aides to assist with personal care activities and maintain dignity.
  • Social Workers to coordinate care resources and provide counseling support.
  • Spiritual Counselors to offer spiritual guidance and support. 
  • Volunteers to provide companionship and respite for caregivers.

Guidance from our supportive counselors extends to the whole family unit. We offer family therapy, coping strategies, and continued bereavement services after a loved one's passing.

Where and How Hospice Care is Provided

Hospice care services from Amavi can be provided wherever the patient calls home - whether that is their private residence, a hospice inpatient facility, hospital, or nursing home. We tailor our care delivery to meet each individual patient's needs and preferences regarding their care setting.  

Our hospice services are available to individuals of all ages, religions, races, and terminal illnesses. Hospice care is also covered under a variety of insurance plans, including Medicare, Medicaid, most private insurers, HMOs, and managed care organizations.

What to Expect When Starting Hospice

As Amavi's hospice team is invited in, a family member or loved one will be designated as the primary caregiver for the patient. This person will collaborate closely with our staff and serve as the main decision-maker for the patient's care.

Our hospice professionals will visit regularly to evaluate the patient, monitor symptoms, and provide personalized care adhering to the customized hospice plan developed for the patient's unique circumstances. The patient's family or caregivers can reach a member of Amavi's hospice team 24 hours a day, 7 days a week should any needs or questions arise.
